Here's the short version. Tirzepatide takes off a lot of weight, and some of that is lean mass. The share of lean mass lost looks ordinary for weight loss, not extra. Resistance training, nutrition and monitoring deserve a separate plan with your clinician.
Does tirzepatide burn muscle?
Some of the weight you lose on tirzepatide is lean mass. In the SURMOUNT-1 body-scan substudy, lean mass fell 10.9% over 72 weeks while fat mass fell 33.9% [1].
The substudy scanned 160 adults from the main SURMOUNT-1 trial with DXA, a low-dose X-ray that splits the body into fat, lean tissue and bone. Everyone had obesity or overweight without diabetes, and 73% were women. The main trial enrolled 2,539 people [2].
| Group | Body weight | Fat mass | Lean mass | Share of loss: fat / lean |
|---|---|---|---|---|
| Tirzepatide, all doses pooled (n=124) | −21.3% | −33.9% | −10.9% | About 75% / 25% |
| Placebo (n=36) | −5.3% | −8.2% | −2.6% | About 75% / 25% |

Zepbound's label puts it the same way: tirzepatide "lowers body weight with greater fat mass loss than lean mass loss" [6].
Is that more lean loss than usual? No
About three-quarters of the weight lost was fat in both the tirzepatide group and the placebo group [1]. The placebo group lost far less weight, but in the same proportions.
The researchers also split people by sex, by age (under 50, 50 to 64, and 65 and over) and by how much weight they lost. The fat-to-lean split stayed similar in most of those groups [1]. That matters for older adults, who have less muscle to spare.
Lean mass isn't the same as muscle
DXA's "lean mass" counts everything that isn't fat or bone mineral: muscle, but also organs, fluids and the water held in fat tissue. A lean-mass drop therefore cannot be converted directly into muscle loss [3].
A 2024 review of GLP-1-based drugs found that MRI studies tell a more reassuring story. Muscle volume fell about as much as you'd expect for the weight lost, and fat stored inside muscle dropped, which may mean better muscle quality [3]. The same review flags older age and more severe illness as reasons to watch muscle closely.
Can you gain muscle while you're on it?
Some people may, but no tirzepatide trial has tested it. The trials measured what happens with diet and activity advice, not with a structured lifting program.
Here's the reasoning, labeled as reasoning. People who are new to lifting, or who have more fat to lose, can sometimes add muscle while eating less. Tirzepatide doesn't change that physiology, but it does cut appetite, which can make eating enough protein harder. Gaining is possible. Keeping is the realistic first goal.
Can you build muscle on Zepbound?
Zepbound is tirzepatide, so the SURMOUNT-1 body-scan result applies: about three-quarters of the weight lost was fat. Keeping muscle on Zepbound depends on training and protein, the same as on any tirzepatide product.
Zepbound is labeled for long-term weight reduction in adults with obesity, or overweight with at least one weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity [6]. Mounjaro is the same molecule, labeled for type 2 diabetes and for lowering the risk of major heart events in adults with type 2 diabetes at high risk [7].
How people keep muscle on tirzepatide
Resistance training and enough protein are the two levers with evidence. Neither needs anything fancy.
- Resistance training. Lifting, machines or bodyweight work gives muscle a reason to stay while you're eating less.
- Protein. A 2018 meta-analysis of 49 trials in 1,863 healthy adults found extra protein increased the muscle and strength gains from resistance training [4]. The boost was smaller with age and larger in people who already trained.
- A plan that fits your appetite. Smaller appetite is the point of the drug. A dietitian can help you hit protein when portions shrink.
One caution on that meta-analysis: its participants weren't on a GLP-1 drug or trying to lose weight. It supports protein plus lifting in general. It doesn't prove the same effect on tirzepatide. Your prescriber or a dietitian sets your targets.
How do you know it's working? The scale can't tell you, because it mixes fat and lean together. Strength is a practical signal: if your lifts hold steady or climb while the weight comes off, you're probably keeping muscle. A body-composition scan gives a clearer picture, but it's worth asking your prescriber whether one is useful for you before paying for it.
Tirzepatide vs semaglutide vs retatrutide on lean mass
No trial has compared these drugs head to head on body composition. Each was scanned in its own studies, with different people and timelines, so the numbers don't line up neatly.
| Drug | What its body-scan studies found | More |
|---|---|---|
| Tirzepatide | About 25% of weight lost was lean mass at 72 weeks, the same share as placebo [1] | This page |
| Semaglutide | Covered in its own guide | Muscle on Ozempic |
| Retatrutide (investigational) | Lean share of weight lost "similar to other obesity treatments" in a phase 2 substudy in type 2 diabetes [5] | Does retatrutide build muscle? |
Retatrutide isn't FDA-approved. Its status is on our retatrutide page.
If you've seen tirzepatide paired with sermorelin for body composition, the evidence for that combination is in sermorelin and tirzepatide together. Questions about tiredness on Zepbound are in does Zepbound give you energy.
If body composition is your goal
Losing fat while keeping muscle is the goal most people mean. Tirzepatide can reduce fat, while strength and lean tissue still need attention during weight loss. Our body composition shortlist lists programs that prescribe for this goal. Not sure which fits? Start the quiz.
