The short version: when a fact on this site was wrong, we fix the page and list the fix here, with what changed and when.

What goes on this log

• A wrong price, plan, state list, pharmacy or policy on a pick page.
• A provider shown as In when it missed a rule, or Not yet when it didn't.
• A study, label fact or FDA status stated incorrectly in a guide.

Routine updates aren't corrections: a provider changing its price, a new study coming out, or a pick moving after a re-read. Those show up as a new month in the page's byline.

How a fix happens

1. You (or one of us) spot something off.
2. It gets checked against the original source: the provider's own site, the study, the label or the FDA.
3. If it was wrong, the page is fixed and an entry goes here within a few days.
