Here's the short version. Retatrutide has a published phase 2 substudy of body composition, and it measured how much lean mass people lost [1]. Nothing in it shows muscle being built. The drug is also still investigational, so anything sold for bodybuilding isn't the product tested in trials [7].
What the body-scan study measured
The human body-composition data come from a phase 2 substudy in 189 adults with type 2 diabetes, scanned by DXA at the start and at week 36 [1]. Of those, 155 had a first scan and 103 finished treatment with both scans.
Participants came from 42 US centers, were 18 to 75 years old and had a BMI of 25 to 50. A bit over half were women. Everyone had type 2 diabetes, so the results may not carry over to people without it [1].
Its main measure was the change in total fat mass. Fat fell sharply at the higher doses and barely moved on placebo or the lowest dose. Eli Lilly, the drug's developer, funded the study, and all seven authors are Lilly employees and shareholders [1].
| Group | Fat mass change from baseline |
|---|---|
| Placebo | −4.5% |
| Dulaglutide 1.5 mg (comparison drug) | −2.6% |
| Retatrutide 0.5 mg | −4.9% |
| Retatrutide 4 mg (pooled) | −15.2% |
| Retatrutide 8 mg (pooled) | −26.1% |
| Retatrutide 12 mg | −23.2% |
Doses are the ones the trial tested. The study summary gives no lean-mass percentages; the authors reported the lean share of weight lost as similar to other obesity treatments [1].
Why "preserves muscle" overstates it
The authors concluded that the proportion of lean mass lost to weight lost was similar to other obesity treatments [1]. That means some lean tissue was lost, as it is with other weight-loss drugs. It wasn't kept, and it wasn't added.
Two details matter here. First, DXA lean soft tissue includes organs and water as well as muscle; bone mineral is measured separately, so a lean-mass drop isn't the same as a muscle drop [4]. Second, across GLP-1 trials the lean share of weight lost ranges from about 15% to as high as 40–60% [4]. A drug that causes more total weight loss can cause a bigger absolute lean loss, even at the same share.
So "similar to other obesity treatments" is reassuring in one way: retatrutide didn't strip a larger share. It's not evidence of muscle preservation beyond the usual.
The cited body-composition report does not establish a strength or physical-function benefit. A scan can show tissue lost, but not whether you can lift, climb stairs or get out of a chair as well as before. Those are the measures that matter most as you age.
Is there any evidence it builds muscle?
No. The muscle-gain claims come from animal studies and personal scan posts, which aren't trials. The cited human data do not establish muscle-building efficacy.
Before trusting a "muscle-sparing" claim, ask three things. Was it a human trial or an animal study? Did it measure muscle itself, or DXA lean mass? And who ran it? For retatrutide, the direct body-composition evidence summarized here is a drug-maker study of lean mass, in people with diabetes.
| Claim | What the evidence shows | Strength of evidence |
|---|---|---|
| Builds muscle | No human study shows muscle gain | None |
| Preserves muscle | Lean share of weight lost similar to other obesity drugs | One sponsor-run substudy, 103 completers |
| Works for bulking | No study; the drug is designed to cut appetite and weight | None |
| Safe for bodybuilding use | Unapproved; products sold for this aren't the trial drug | FDA status statement |
What retatrutide is and where it stands
Retatrutide is a weekly injection that acts on three hormone receptors: GIP, GLP-1 and glucagon. It has finished a phase 3 weight trial but isn't FDA-approved.
In TRIUMPH-1, adults with obesity and without diabetes lost 25.0% of body weight on 12 mg over 80 weeks, against 3.9% on placebo [2]. That trial reported weight, knee pain and sleep apnea results, not muscle. The earlier phase 2 obesity trial set up those doses [3].
"Bulking" or bodybuilding on retatrutide
Using an appetite-suppressing weight-loss drug to bulk works against the calorie surplus that gaining muscle usually needs. The products sold for this are unapproved, too.
FDA has warned telehealth companies for marketing retatrutide and ingredient distributors for selling it [7]. Vials labeled "research use only" don't change that status. For where peptide pairings stand legally, see the legal status of combining tesamorelin with other peptides.
What keeps muscle during weight loss
Progressive resistance training and enough protein are what trials show build or keep muscle in adults. In a meta-analysis of 49 trials with 1,863 people, protein supplements added to resistance training increased gains in strength and fat-free mass [6].
The added gain was modest, smaller with age and larger in people who already trained [6]. Your prescriber or a registered dietitian sets the protein target and plan that fit you.
- Lift or do resistance work on a regular schedule your clinician agrees with.
- Ask how much protein makes sense for your weight, age and kidneys.
- If you're on a weight-loss drug, ask whether a body-composition scan is worth doing.
Approved options with body-composition data
Approved weight drugs have their own body-scan data. In a SURMOUNT-1 substudy, about 75% of the weight lost on tirzepatide was fat and about 25% was lean mass, the same split as placebo [5]. Lilly funded that study too.
If muscle is your goal, start with what tirzepatide trials show about muscle or what semaglutide trials found. The body-composition shortlist covers the approved routes. For the other big retatrutide question, see whether it affects energy.
