Here's the short version. Tirzepatide, sold as Zepbound and Mounjaro, isn't an energy drug. Tiredness shows up in its side-effect table. Feeling better later, for those who do, tracks with how much weight they lose. Both things can be true for the same person at different points, and your prescriber is the one to sort out which is which.
What Zepbound is approved for
Zepbound is approved, with a reduced-calorie diet and more activity, to reduce excess body weight and keep it off long term. That covers adults with obesity, or overweight plus at least one weight-related condition. It's also approved to treat moderate-to-severe obstructive sleep apnea in adults with obesity [1].
Mounjaro is the same molecule, approved to improve blood-sugar control in type 2 diabetes. It's also approved to lower the risk of major heart events in adults with type 2 diabetes at high risk for them [2]. The energy answer is the same for both. Their labels report fatigue from different trials, so the numbers below are kept apart.
Compounded tirzepatide isn't FDA-approved, and none of the trial results on this page were run on compounded versions.
Does tirzepatide give you energy?
It is not approved as an energy treatment or stimulant, and fatigue appears in its side-effect table. On the Zepbound label, fatigue is one of the most common side effects, reported in at least 5% of people [1].
| Placebo | 5 mg | 10 mg | 15 mg | |
|---|---|---|---|---|
| People in the trials | 958 | 630 | 948 | 941 |
| Fatigue (includes asthenia, fatigue, lethargy, malaise) | 3% | 5% | 6% | 7% |
Pooled from two placebo-controlled trials of up to 72 weeks [1]. Dose columns are trial arms, not advice; your prescriber sets your dose.
Mounjaro's label reports fatigue a different way. In its heart-outcomes trial of adults with type 2 diabetes, run against another diabetes drug rather than placebo, 10% of the 6,647 people on Mounjaro reported fatigue [2]. That trial ran for years, so the figure isn't comparable with the Zepbound table.
How common is fatigue, really?
Less common than you might expect, but real. Picture 100 people on the highest dose tested: about 7 reported fatigue, against about 3 of 100 on placebo. That's roughly 4 extra people per 100 [1].
Put the other way, more than nine in ten people on Zepbound in those trials didn't report fatigue. And some of the tiredness on placebo shows how often people feel worn out anyway, drug or not.
Side effects of any kind made 4.8% to 6.7% of people on Zepbound stop for good, depending on dose, against 3.4% on placebo [1]. The label doesn't break that down by fatigue alone.
Why Zepbound can make you tired
The label lists fatigue without giving a cause. The usual clinical explanations are simple: you're eating a lot less, and stomach side effects can cut into food and fluids.
That's reasoning, not a trial result. But it fits the pattern. Nausea, diarrhea, vomiting and constipation top the label's list of common side effects. Most people who stopped Zepbound because of side effects did so in the first few months, mainly for stomach problems [1].
Why some people feel better later
In SURMOUNT-1, people who lost more weight reported larger improvements in physical function and quality-of-life scores.
Every tirzepatide group improved more than placebo on the questionnaires. The gains grew with weight lost, starting from about 5% weight loss, and the group that lost 20% or more saw the biggest changes by week 72 [3].
A second look at three SURMOUNT trials found that people who started with the lowest physical-function scores improved the most, even though weight loss was similar across groups [4]. Both analyses were written mostly by employees of Eli Lilly, which makes the drug.
SURMOUNT-1 also measured the SF-36v2 vitality domain: scores improved by 2.33 to 3.19 points across tirzepatide arms versus 0.21 with placebo at week 72 [3]. That is a patient-reported vitality result, not proof of a direct stimulant effect.
Sleep apnea and daytime tiredness
Zepbound is approved to treat moderate-to-severe sleep apnea in adults with obesity. In its two sleep apnea trials, breathing interruptions fell by about 20 to 24 more events an hour than on placebo after a year [5].
Those trials also reported better scores on sleep-related questionnaires about sleep problems and how sleep affects the day [5]. If sleep apnea is part of your picture, that's worth raising with your prescriber, and the sleep and recovery shortlist covers the goal more broadly.
What to do about fatigue
Tell your prescriber. They'll look at how much you're eating and drinking, whether a recent dose change lines up with it, and whether something else is behind it.
Don't skip or change doses on your own. Your prescriber and the label set your schedule, and they can advise you about missed doses and restarting.
If you're on the semaglutide side instead, does Wegovy give you energy covers that drug's label. Mood questions have their own page too: does Zepbound help with anxiety.
If energy is your goal
Tirzepatide isn't the tool for energy on its own. If weight is weighing you down, it may help indirectly, and keeping muscle matters along the way: see can you build muscle on tirzepatide. Some people also ask about inflammation or pairing it with other treatments, like sermorelin and tirzepatide together.
For energy as the main goal, start with the energy shortlist, or take the 30-second match to sort options by what you want.
