Here's the short version. Tirzepatide lowers the blood markers doctors use to track inflammation, and the drop gets bigger at higher trial doses. That's not the same as treating an inflammatory condition, and neither label says it is. Joints are where the evidence gets thin.
What Zepbound and Mounjaro are approved for
Zepbound is approved to reduce and keep off excess weight and to treat moderate to severe obstructive sleep apnea in adults with obesity. Mounjaro is approved for blood-sugar control in type 2 diabetes and to cut heart risk in some of those patients.
| Zepbound | Mounjaro | |
|---|---|---|
| Drug inside | Tirzepatide | Tirzepatide |
| Weight | Reduce excess weight and keep it off, in adults with obesity, or overweight plus a weight-related condition [1] | Not a labeled use |
| Sleep apnea | Moderate to severe obstructive sleep apnea in adults with obesity [1] | Not a labeled use |
| Type 2 diabetes | Not a labeled use | Blood-sugar control in adults and children 10 and older [2] |
| Heart risk | Not a labeled use | Lower risk of major heart events in adults with type 2 diabetes at high risk [2] |
| Inflammation or arthritis | Not a labeled use | Not a labeled use |
Neither label implies the trials below were run for inflammation. Compounded tirzepatide isn't FDA-approved and wasn't the product tested.
hsCRP fell by up to 55% in the big weight-loss trial
In SURMOUNT-1, hsCRP fell 36.9 to 54.6% more than placebo by dose at 72 weeks, and IL-6 fell 25 to 30%.
SURMOUNT-1 was the main 72-week Zepbound trial in adults with obesity or overweight [4]. A 2026 analysis selected 100 treatment completers per group and analyzed 392 after excluding low-volume samples. Samples came from the start, week 24 and week 72 [3]. The table shows the change against placebo at week 72.
| Marker | 5 mg arm | 10 mg arm | 15 mg arm |
|---|---|---|---|
| hsCRP [3] | −36.9% | −46.9% | −54.6% |
| Interleukin-6 | −25.4% | −27.8% | −30.2% |
| White blood cells | Not significant | −8.6% | −10.0% |
| E-selectin (blood-vessel lining marker) | −12.6% | −20.0% | −26.4% |
Trial arms, not a dose guide. Analysis funded by Eli Lilly, whose employees helped design, analyze and write it.
An earlier signal came from a phase 2 diabetes trial. There, the 15 mg arm lowered hsCRP compared with placebo, but not compared with dulaglutide, an older GLP-1 drug. IL-6 didn't change in that trial. The authors are Lilly employees [5]. For the semaglutide side of this question, see does semaglutide help with inflammation?
How tirzepatide lowers inflammation: the trials don't isolate it
The trials don't pin down a mechanism. The marker changes moved with weight loss, lower insulin resistance and less liver stress.
In the same SURMOUNT-1 analysis, insulin resistance (HOMA-IR) fell 26 to 39% against placebo. Leptin, a hormone made by fat tissue, fell 44 to 61%. GGT, a liver-stress marker, fell 19 to 33%. Across the tirzepatide groups, marker changes were checked against weight change [3]. The authors describe associations. They don't claim tirzepatide acts on the immune system directly, and this page doesn't either.

Joint pain and arthritis: one add-on trial, one still running
Tirzepatide hasn't been tested alone for arthritis or joint pain. One published trial added it to a psoriatic arthritis drug, and a knee-osteoarthritis trial is still under way.
TOGETHER-PsA enrolled 271 adults with active psoriatic arthritis and overweight or obesity. Everyone got ixekizumab, a psoriatic arthritis medicine; half also got tirzepatide. On a key secondary arthritis measure, a 50% improvement in ACR response criteria (ACR50), 33.5% on the pair got there vs 20.4% on ixekizumab alone. Lilly scientists are among the authors [7]. Its primary combined endpoint, ACR50 plus at least 10% weight reduction, was reached by 31.7% versus 0.8%. These results concern that specific combination and disease. It doesn't show tirzepatide treats arthritis by itself.
A ClinicalTrials.gov search can help locate knee-osteoarthritis studies, but a registration is not a demonstrated treatment benefit [9]. The published arthritis result discussed above used tirzepatide together with ixekizumab in psoriatic arthritis; it does not establish tirzepatide alone for ordinary knee pain. The Zepbound label doesn't list joint pain among its common side effects [1]. Losing weight takes load off knees, which your prescriber can weigh for you.
Sleep apnea is the one labeled use with an inflammation link
The sleep-apnea trials included inflammation among their measured secondary outcomes; that does not create a general inflammation indication.
SURMOUNT-OSA ran two 52-week trials in adults with obesity and moderate to severe sleep apnea. Breathing pauses per hour (AHI) fell by 25.3 vs 5.3 in the first trial and 29.3 vs 5.5 in the second. hsCRP was one of the key secondary results, and it fell more on tirzepatide than on placebo. Eli Lilly funded both trials [6]. If better sleep is the real goal, see our sleep and recovery shortlist.
What these marker results can't tell you
They show blood tests moving. They don't show fewer flare-ups, less pain or faster recovery.
- Post hoc. The SURMOUNT-1 marker study was planned after the trial, not as one of its main questions [3].
- Finishers only. It sampled people who completed treatment, so people who stopped early aren't in those numbers [3].
- One sponsor. Lilly funded the analyses and its employees helped write them [3] [5].
- Markers, not outcomes. A lower hsCRP isn't the same as fewer heart attacks or less joint pain, and this analysis didn't count either.
How quickly the markers moved: within weeks, at set check-ins
Trials checked at set points. In a phase 2 diabetes trial, hsCRP was measured at 4, 12 and 26 weeks, and it was already dropping by week 4 on the two higher doses.
In that trial, hsCRP and two other markers "rapidly decreased within 4 weeks" on the 10 and 15 mg arms. By week 26, the 15 mg arm was lower than placebo [5]. SURMOUNT-1 checked at weeks 24 and 72 [3]. These biomarker-trial checkpoints do not establish a general symptom timetable. The separate PsA combination trial reported early ACR50 separation, which is specific to that disease and combination. Your prescriber sets your schedule.
Stopping before surgery: your care team decides
The label tells patients to tell their care team before any planned surgery or procedure. Your surgeon and anesthesia team decide what to do.
Both labels warn that rare cases of food getting into the lungs under anesthesia have been reported with GLP-1 drugs [1] [2]. Several medical societies published joint guidance in 2024 on GLP-1 drugs around procedures [8]. Your team applies it to your case.
If inflammation or recovery is your goal
Tirzepatide does not have a general inflammation indication. A prescriber can tell you whether it fits your weight, blood-sugar or sleep-apnea picture.
