Here's the short version. Semaglutide lowers CRP, the blood marker doctors use to gauge inflammation, and the drop is large and consistent across trials. There is no general inflammation, arthritis or autoimmune indication; Wegovy has a specific accelerated MASH indication. A lower marker and a treated disease are two different claims.
What Ozempic and Wegovy are approved for
Ozempic is approved for blood sugar in type 2 diabetes and for heart and kidney risk in some of those patients. Wegovy is approved for weight, heart-risk reduction and a liver condition called MASH.
| Drug | Form | Approved uses (adults unless noted) |
|---|---|---|
| Ozempic [1] | Weekly injection | Blood-sugar control in type 2 diabetes, with diet and exercise |
| Ozempic [1] | Weekly injection | Lower risk of major heart events in type 2 diabetes with heart disease |
| Ozempic [1] | Weekly injection | Lower risk of kidney decline, kidney failure and heart death in type 2 diabetes with chronic kidney disease |
| Wegovy [2] | Weekly injection or daily tablet | Lower risk of major heart events in heart disease with obesity or overweight |
| Wegovy [2] | Weekly injection or daily tablet | Reduce excess weight and keep it off (the injection also from age 12) |
| Wegovy [2] | Weekly injection | Noncirrhotic MASH with moderate to advanced liver fibrosis (accelerated approval) |
No general inflammation, arthritis or psoriasis indication; the specific Wegovy MASH indication is separate.
Neither list establishes a general inflammation or arthritis indication. This page reports what trials of the brand drugs measured. Compounded semaglutide isn't FDA-approved and wasn't the product tested.
CRP fell by about 40% in every major trial
In trials, semaglutide lowered CRP by 39 to 48% more than placebo across STEP 1 to 3, and hsCRP by 37.8% in SELECT.
STEP 1, 2 and 3 were 68-week weight-loss trials in adults with overweight or obesity. STEP 2 enrolled people with type 2 diabetes. At week 68, CRP was lower than placebo by 44% in STEP 1, 39% in STEP 2 and 48% in STEP 3. The drop held regardless of starting weight or blood-sugar status [3].
SELECT tested semaglutide in 17,604 adults with heart disease and overweight or obesity, but no diabetes. hsCRP fell 37.8% by week 104, and the drop showed up across every starting hsCRP level [4]. Novo Nordisk, which makes the drug, funded these trials [5], and its employees are among the authors [3] [4].
Weight loss explains most of it, but not all
Mostly the two move together. But in SELECT, hsCRP started falling by weeks 4 to 8, before major weight loss, and it fell in people who didn't lose weight.
In the STEP trials, the CRP drop ran in parallel with weight loss and lined up with smaller waists, lower fasting insulin and lower insulin resistance [3]. SELECT's analysis found the same link with weight, plus the early drop. The authors' modeling suggests lower inflammation may explain part of the heart benefit in SELECT [4].
That's an association. Neither paper shows how semaglutide lowers CRP, and the authors don't claim to. For the same question about the other big GLP-1 drug, see does tirzepatide help with inflammation?
| Trial (year) | Who | What changed vs placebo | Funding |
|---|---|---|---|
| STEP 1, 2 and 3, CRP analysis (2023) [3] | Adults with overweight or obesity, with or without type 2 diabetes | CRP down 44%, 39% and 48% at 68 weeks | Novo Nordisk |
| SELECT hsCRP analysis (2026) [4] | Adults with heart disease and overweight or obesity, no diabetes | hsCRP down 37.8% at 104 weeks; seen by weeks 4 to 8 | Novo Nordisk (SELECT) [5] |
| STEP 9 (2024) [6] | Adults with knee osteoarthritis and obesity | WOMAC pain down 41.7 vs 27.5 points; SF-36 physical function up 12.0 vs 6.5 | Novo Nordisk |
All three tested the brand drug at weight-management doses. None tested compounded semaglutide.
What a lower CRP can and can't tell you
A lower CRP is a sign of less inflammation in the blood. It isn't proof that any one condition got better.
In SELECT, people who started with higher hsCRP had more heart attacks, strokes and heart deaths later. Bigger hsCRP drops on semaglutide went with lower risk [4]. That makes CRP useful for heart risk. It doesn't make semaglutide a treatment for joint, skin or gut inflammation, which each need their own trials. CRP is also a general marker. It can't say where in the body the inflammation is.
Knee osteoarthritis pain eased in one trial
In knee osteoarthritis with obesity, semaglutide cut WOMAC pain scores by 41.7 points vs 27.5 on placebo over 68 weeks.
STEP 9 enrolled 407 adults with obesity and moderate knee osteoarthritis, most of them women. Everyone also got diet and activity counseling. Weight fell 13.7% on semaglutide vs 3.2% on placebo. Physical function scores rose more too [6]. WOMAC is a 0 to 100 pain scale where lower is better, so placebo plus counseling helped a lot as well.
That trial was about knee osteoarthritis, a wear-and-load condition. It doesn't tell you about rheumatoid arthritis, which is autoimmune. A ClinicalTrials.gov search listed no semaglutide trial in rheumatoid arthritis [9].
Psoriasis, Crohn's and lupus: early evidence only
None is a labeled use, and the published evidence is early.
A 2026 review from the National Psoriasis Foundation's medical board found psoriasis scores fell in studies of GLP-1 drugs. But most of those studies had 7 to 48 patients, lasted 6 months or less and had no control group. The authors said definitive conclusions await larger randomized trials [7]. Randomized psoriasis trials of semaglutide are listed on ClinicalTrials.gov, without treating a registry listing as a positive trial result [9]. Crohn's disease and lupus aren't labeled uses either, so ask your specialist before reading anything into stories online.
Does semaglutide weaken the immune system? Not on the label
Neither label describes semaglutide as suppressing the immune system. The trials report CRP falling, which is a marker of inflammation, not a measure of how well your immune system fights infection.
The labels discuss immunogenicity, including antibodies some people make against the drug itself [1] [2]. If you take medicines that do suppress immunity, your prescriber should know about every drug you're on.
Stopping before surgery: your care team decides
The label tells patients to tell their care team before any planned surgery or procedure, because semaglutide slows stomach emptying. Your surgeon and anesthesia team decide what to do.
Both labels warn that rare cases of food in the lungs during anesthesia have been reported with GLP-1 drugs [1] [2]. Several medical societies published joint guidance in 2024 on GLP-1 drugs around procedures [8]. Your team uses that, plus your own case. If inflammation or recovery is your goal
A prescriber can tell you whether a GLP-1 fits. The labels do not establish a general inflammation indication; the diagnosis and exact proposed use need clinical review.
If weight, blood sugar or heart risk is part of your picture, start with our metabolic shortlist. If recovery and sleep are the real goal, the sleep and recovery shortlist is a better fit.
