The short answer

Semaglutide is a GLP-1 drug with approved injection and oral formulations. Indications depend on the product and route. STEP 1 found 14.9% average weight loss over 68 weeks with weekly 2.4 mg in adults without diabetes. Compounded products are not FDA-approved; cost comparisons need the exact product, supply and charge schedule.

Definition

Semaglutide is a lab-made version of GLP-1, a hormone your gut releases after you eat. It acts on GLP-1 receptors in the brain, stomach and pancreas, so you feel full sooner and your blood sugar stays steadier. Novo Nordisk sells it as Ozempic and Wegovy.

Here's the short version. Semaglutide is the drug behind Ozempic and Wegovy, and it has the big outcome trials to back it. Withdrawal can lead to substantial regain, side effects can be significant, and compounded products are not the formulations tested in the cited outcome trials. If a steadier metabolism is your goal, start with what's approved, then look at price.

How semaglutide works: it makes your brain think you've eaten

Semaglutide is built to look like GLP-1, with 94% of the same amino-acid sequence [5]. Your own GLP-1 lasts minutes. Semaglutide lasts days, which is why one shot covers a week.

It works in three places. In the brain, GLP-1 receptors sit in the areas that control appetite, and the label calls GLP-1 "a physiological regulator of appetite and caloric intake" [5]. In the stomach, it slows how fast food moves on. The label says it plainly: "Semaglutide delays gastric emptying" [5]. In the pancreas, it nudges insulin up when blood sugar is high.

Those same effects explain the side effects. A slower stomach means fullness, but also nausea and constipation for some people. For more on that, see does Ozempic slow digestion. For what it does to cravings and mood, see what Ozempic does to your brain.

Ozempic, Wegovy and the Wegovy pill are the same molecule with different labels

These selected products share semaglutide but differ in formulation, route, dosing and approved populations. They are not automatically interchangeable, and an injection indication should not be applied to a tablet.

Semaglutide brands and what each is approved for
BrandFormApproved forFirst approved
OzempicWeekly injection penBlood sugar in type 2 diabetes; lowering heart attack, stroke and heart-death risk in type 2 diabetes with heart disease; lowering kidney-failure risk in type 2 diabetes with kidney disease [6]December 5, 2017 [2]
Wegovy injectionWeekly injection pen (Wegovy HD is the highest dose)Long-term weight reduction in adults and teens 12 and up; heart-risk reduction in adults with heart disease and obesity or overweight; Noncirrhotic MASH in adults with F2 to F3 fibrosis (accelerated approval) [5]June 4, 2021 [1]
Wegovy tabletsDaily pillLong-term weight reduction and heart-risk reduction in adults [5]December 22, 2025 [3]

Selected formulations are shown, not an exhaustive semaglutide catalog. The cited Ozempic injection label is not a weight-management indication.

On dosing, the label sets the range and your prescriber sets the plan. The selected injection label includes a 0.25 mg weekly initiation strength and 7.2 mg Wegovy HD for selected adult weight-management use; dose eligibility differs by product and indication [5]. Higher doses come on slowly, and the steps between belong to your prescriber and the label.

What the trials show: about 15% weight loss and fewer heart events

Semaglutide has some of the largest trials of any weight drug. People lost about 15% of their weight, and in people with heart disease, major heart events fell by a fifth. The results came from the brand drug at labeled doses, not from compounded versions.

SELECT, 2023 [13]17,604 adults 45 and older with heart disease and overweight, no diabetesHeart attacks, strokes and heart deaths: 6.5% vs 8.0% on placebo, a 20% lower risk
SUSTAIN-6, 2016 [14]3,297 adults with type 2 diabetes at high heart riskFewer major heart events (6.6% vs 8.9%), but more diabetic eye complications

STEP-HFpEF, 2023 [16]529 adults with a common type of heart failure and obesityBetter symptom scores, longer walking distance, 13.3% weight loss vs 2.6%
ESSENCE, 2025 [17]Adults with MASH and F2 to F3 liver scarring (first 800 of 1,197)MASH resolution without fibrosis worsening: 62.9% vs 34.3% at 72 weeks; fibrosis improvement without MASH worsening: 36.8% vs 22.4%
SURMOUNT-5, 2025 [18]751 adults with obesity, head to head with tirzepatideSemaglutide 13.7% weight loss vs 20.2% on tirzepatide at 72 weeks

Every trial above tested brand semaglutide. None tested a compounded version. Most were funded by the maker; SURMOUNT-5 was funded by the maker of tirzepatide.

The big caveat is what happens after. In the STEP 1 extension, people regained 11.6 of the 17.3 percentage points they had lost within a year of stopping [12]. Those are group averages from a selected exploratory subset, not a prediction that every person regains the same amount. Discuss maintenance and any discontinuation plan with the prescriber.

Weight isn't the whole story. Part of what you lose is lean tissue, which matters more as you age. Can you build muscle on Ozempic covers what the scans show and how training changes it. If you're comparing drugs, the cited obesity head-to-head study favored tirzepatide for weight loss at the studied doses; see what tirzepatide is.

Semaglutide and longevity: disease outcomes differ from healthy lifespan

The cited trials do not establish longer lifespan in otherwise healthy, normal-weight people. What the trials do show is fewer heart and kidney events in people who already had disease, which is a different claim.

  • Heart. SELECT enrolled people with existing heart disease. Its primary endpoint was a cardiovascular composite; mortality was also assessed, but this is not a general healthy-population lifespan trial [13].
  • Kidneys. In FLOW, deaths from any cause were also lower (hazard ratio 0.80), in people with diabetes and kidney disease [15].
  • Inflammation. Semaglutide lowers CRP, a blood marker of inflammation. That's covered in does semaglutide help with inflammation.
  • Energy. It isn't a stimulant, and fatigue is a listed side effect. See does Wegovy give you energy.

Side effects: mostly stomach trouble, plus a few serious warnings

Nausea, diarrhea, vomiting and constipation are the most common side effects, and they tend to show up as the dose goes up. A smaller number of people get serious problems, which is why semaglutide is a prescription drug with follow-up.

  • Boxed warning. Semaglutide caused thyroid C-cell tumors in rodents. It's unknown whether it does in people, and it isn't for anyone with a personal or family history of medullary thyroid cancer or MEN 2 [5].
  • Common (5% or more). Nausea, diarrhea, vomiting, constipation, belly pain, headache, fatigue, indigestion, dizziness, bloating, burping, gas, reflux and hair loss, plus low blood sugar in type 2 diabetes [5].
  • Label warnings. Pancreatitis, gallbladder disease, kidney injury from dehydration, severe stomach reactions, allergic reactions, a faster heart rate, and diabetic eye complications [5].
  • Before surgery. Food can stay in the stomach under anesthesia or deep sedation. Tell the surgical team you take it [5].
  • Why people stopped. In STEP 1, 4.5% quit over stomach side effects, vs 0.8% on placebo [11].

FDA status: the brands are approved, and compounding narrowed after the shortage

Ozempic and Wegovy are FDA-approved. Compounded semaglutide is not: FDA doesn't review compounded drugs for safety, effectiveness or quality before they're sold [8]. Shortage status affected particular compounding conditions and enforcement policies; it did not waive all manufacturing, prescribing or quality requirements. The shortage resolution and subsequent transition dates narrowed those policies, while patient-specific legal conditions still require assessment.

WhenWhat happenedWhat it means for you
December 5, 2017FDA approved Ozempic for type 2 diabetes [2]The molecule reached the market
June 4, 2021FDA approved Wegovy for chronic weight management [1]A weight-loss label at higher doses
March 8, 2024Wegovy approved to lower heart attack, stroke and heart-death risk in adults with heart disease and obesity or overweight [1]First weight drug with a heart-outcome label
February 21, 2025FDA declared the semaglutide injection shortage resolved [7]The shortage-era allowance for copies began to wind down
April 24, 2025A federal court denied an injunction; FDA said its grace period for state-licensed pharmacies had ended. Outsourcing facilities had until May 22, 2025 [7]Shortage-dependent enforcement discretion ended on the stated schedules
August 15, 2025Wegovy approved for MASH with moderate to advanced fibrosis, under accelerated approval [1]A liver indication, pending a confirmatory trial
December 22, 2025FDA approved Wegovy tablets [3]A daily pill option for weight
April 7, 2026A generic semaglutide injection received tentative approval [4]Not for sale yet. Tentative approval isn't final approval
April 30, 2026FDA proposed keeping semaglutide off the list of bulk ingredients outsourcing facilities may use, citing no clinical need [9]Proposal concerns bulk-ingredient eligibility under 503B, not every preparation pathway

What's allowed now. A state-licensed pharmacy may not regularly make what FDA calls "essentially a copy" of an approved drug: the same ingredient, in a similar strength, by the same route. A prescriber can document a significant difference for a specific patient, and FDA describes enforcement discretion concerning the regularly-or-inordinate-amounts condition when four or fewer prescriptions of that compounded product are filled in a calendar month. That is not blanket permission or a universal legal production ceiling [7]. FDA’s example specifically compares semaglutide plus B12 with the respective commercially available products at same/similar/easily substitutable strengths and the same route; adding B12 does not automatically avoid the copy analysis [7].

Two more things to know. FDA says salt forms such as semaglutide sodium and semaglutide acetate are different ingredients from the approved drug, with no known lawful basis for compounding [8]. And as of May 31, 2026, FDA had 990 adverse-event reports involving compounded semaglutide; reports do not by themselves establish causation or an incidence rate [8]. Ask any program which form of semaglutide it uses and which pharmacy makes it.

Compare the actual product and payment period

NovoCare’s selected standard Wegovy offer is $349 per manufacturer-defined month, meaning a four-pen box. The page describes a $199 introductory offer for each of two eligible starting-strength fills, subject to its date and eligibility conditions; HD and tablets have separate prices. These are manufacturer medication offers, not proof of a calendar-month clinical subscription. Confirm the current offer and whether care, shipping or program membership adds a charge [10].

Strut Health$129 per thirty daysAnnualized monthly comparison: $130.79; selected auto-refill preparation, not approved Wegovy
Brightmeds$129 on the one-month subscriptionSelected current offer and eligibility conditions; a twenty-eight-day supply does not establish a twenty-eight-day charge

Selected examples, not a complete market survey or a claim of equivalent safety/effectiveness [19] [20]. Obtain the exact prescription, total, renewal date and refund cutoff.

Use the billing calculator to distinguish fixed-day and calendar-month charges and the provider checklist to retain the complete quote. Membership, labs, visits and prepaid commitments can change the total. Coverage must be confirmed separately for medication and clinical care; a savings offer is not insurance coverage.

How to get semaglutide prescribed

You need a prescriber who agrees it fits your health and your goal. For Wegovy, that usually means obesity, or overweight with a weight-related condition. Telehealth programs run the intake online.

  1. A health history. Expect questions on thyroid cancer in the family, pancreatitis, gallbladder problems, diabetes medicines and planned surgery.
  2. A clinician visit. Often a form plus a video or message visit. Some programs ask for recent labs.
  3. A choice of product. Brand Wegovy or Ozempic through a pharmacy, or a compounded version through the program's pharmacy. Ask which one you're getting.
  4. Follow-up. The dose goes up slowly. A good program checks in before each change and tells you who to call about side effects.

Five questions to ask before you pay

A program worth your time answers these up front. If it can't, it isn't ready for you yet.

  1. Is this brand Wegovy or Ozempic, or a compounded product? If compounded, is it semaglutide base or a salt form?
  2. Which pharmacy makes it, and is that pharmacy licensed in my state?
  3. Does the price change as my dose goes up, and what will month three cost?
  4. If it ships in a vial, how is each dose measured, and who checks that I'm doing it right?
  5. What happens to a prepaid plan if FDA's compounding rules change?

Who semaglutide fits, and who should look elsewhere

  • Worth asking about if you have obesity, or overweight with a condition like high blood pressure, prediabetes or heart disease, and you're ready to stay on a drug long term.
  • Look elsewhere if you have a personal or family history of medullary thyroid cancer or MEN 2, or product hypersensitivity. A pancreatitis history or severe stomach disorder requires an individual clinician assessment; these are not all the same as a labeled absolute contraindication.
  • Ask about tirzepatide if weight loss is the main goal. It produced more weight loss head to head. See tirzepatide.
  • Not a longevity shortcut. The cited disease-outcome trials do not establish a general longevity benefit in otherwise healthy, normal-weight people. Metformin gets asked about for the same reason, with its own gaps.

Curious about what's next? Retatrutide is an investigational drug in the same family, not approved for routine commercial supply; authorized research and expanded-access pathways have separate requirements. Weighing weight goals alongside other compounds? Does sermorelin help with weight loss answers a common one.

Frequently asked

Is semaglutide the same as Ozempic?
Yes. Semaglutide is the drug inside Ozempic, Wegovy and the Wegovy pill. Ozempic is approved for type 2 diabetes. Wegovy is approved for weight reduction, heart-risk reduction and a liver disease called MASH.
Is compounded semaglutide FDA-approved?
No. FDA doesn't review compounded drugs before they're sold. The shortage that let pharmacies make copies ended on February 21, 2025, and pharmacies now face limits on making copies of the brand.
Is there a generic semaglutide?
The cited generic injection application has tentative approval rather than final marketing approval. That application status does not authorize commercial sales; it should not be confused with an approved generic product.
How much weight do people lose on semaglutide?
In the STEP 1 trial, adults without diabetes lost 14.9% of their body weight over 68 weeks, compared with 2.4% on placebo. A selected extension subset regained about two-thirds of prior loss after medication and structured lifestyle support stopped.
What are the most common side effects of semaglutide?
Nausea, diarrhea, vomiting, constipation and belly pain are the most common. The label also warns about pancreatitis, gallbladder disease and thyroid tumors seen in rodents.
How much does semaglutide cost without insurance?
The selected NovoCare offer defines a month as a four-pen box, with standard and introductory medication prices subject to eligibility. Clinical programs may add care or membership. Confirm actual supply, today’s charge, subsequent amount and renewal interval.
