The short answer
Enclomiphene is an oral SERM studied in men with secondary hypogonadism. It can raise testosterone while preserving sperm concentration in the cited trials, but clinical symptom and fertility benefits remain uncertain. Standalone enclomiphene is not FDA-approved; Category 1 compounding policy is conditional, and the 2022 advisory vote was not a final rule.
Definition
Enclomiphene (enclomiphene citrate) is a selective estrogen receptor modulator (SERM) taken by mouth. It's the trans isomer of clomiphene, and it raises the pituitary hormones that tell the testes to make testosterone and sperm. It's sold in the US as a compounded capsule, not as an approved drug.
Key facts
| Fact | Detail |
|---|---|
| Class | Selective estrogen receptor modulator (SERM) |
| FDA status | Not FDA-approved. No standalone enclomiphene active-ingredient product is approved in the reviewed FDA record. Listed in Category 1, "under evaluation", on FDA's 503A list updated May 14, 2026[4] |
| FDA advisers | Voted 4 yes, 8 no on adding it to the permanent compounding list (June 8, 2022)[3] |
| Use it was studied for | Raising testosterone, LH and FSH in men with secondary hypogonadism, low T that starts at the brain[3] |
| Related approved drug | Clomiphene (Clomid), a mix of enclomiphene and zuclomiphene, approved for ovulation problems in women[7] |
| Form online | Compounded oral capsule |
| Controlled substance | No. It isn't on DEA's controlled substances list; testosterone is[8] |
| Cost comparison | Medication, monitoring and any required membership or prepaid commitment |
| Who prescribes | A licensed clinician after hormone labs, usually testosterone, LH and FSH |
Here's the short version. Enclomiphene is the pill men ask about when they want more testosterone and still want to keep their fertility. It works through your own hormones instead of replacing them. The catch: it has never been approved, its trials were small and mostly run by its developer, and its legal footing for compounding could change. If hormones are your goal, read the status first, then the price.
Enclomiphene raises your own testosterone, it doesn't replace it
Enclomiphene works upstream. It blocks estrogen's feedback signal at the brain, so the pituitary sends out more LH and FSH. LH tells the testes to make testosterone. FSH supports sperm production.
That's the opposite of testosterone therapy. When testosterone comes from outside, the brain reads the higher level and turns LH and FSH down. Your own production, and often sperm production, drops while you're on it. Enclomiphene leaves that loop running.
Where it comes from: clomiphene, the fertility pill sold as Clomid, is a blend of two geometric isomers. The label describes it as "a mixture of two geometric isomers [cis (zuclomiphene) and trans (enclomiphene)]" with 30% to 50% of the cis form [7]. Enclomiphene is the isolated trans isomer; Clomid is not necessarily a fifty-fifty mixture. Clomid's approval covers women with ovulation problems. It doesn't cover men, and it doesn't cover enclomiphene by itself.
Who it was studied for: men with low T that starts at the brain
The trials enrolled men with secondary hypogonadism: low testosterone with LH that's low or not as high as it should be. That's the use FDA's advisers reviewed in 2022 [3].
In the two phase III trials, men were 18 to 60, overweight, and had morning testosterone at or below 300 ng/dL with LH under 9.4 IU/L, measured twice [11]. That's a narrower group than the "low T" ads aim at. Men over 60, men with testicular failure, and women weren't in these studies.
Some clinicians prescribe clomiphene to men off-label for the same reason. The Kaminetsky study notes that clomiphene "is employed off-label in men who have low testosterone" [9]. Off-label clomiphene is an FDA-approved drug used outside its label. Compounded enclomiphene is an unapproved drug made by a pharmacy. Those aren't the same thing, so ask which one you'd be getting.
What the studies show: testosterone up, sperm kept, symptoms unproven
Across small trials, enclomiphene raised testosterone to levels close to testosterone gel while keeping sperm counts in the normal range. The regulator-reviewed trials did not establish clinically meaningful symptom improvement; hormone normalization is a different endpoint.
| Study | Who took part | What it found |
|---|---|---|
| Kaminetsky 2013, phase IIB [9] | 12 men with secondary hypogonadism who had used testosterone gel; open-label | Testosterone rose on both drugs over 6 months. Sperm counts rose in 7 of 7 men on enclomiphene at 3 months and 6 of 6 at 6 months. Testosterone fell back to baseline a month after stopping |
| Wiehle 2014, phase II [10] | Men with secondary hypogonadism, vs 1% testosterone gel | Testosterone, estradiol and LH rose about as much as on gel. FSH and LH rose on enclomiphene; sperm counts were conserved |
| Kim 2016, two phase III trials [11] | Overweight men 18 to 60, 16 weeks, double-blind, vs AndroGel 1.62% and placebo | Testosterone rose in every active group. LH and FSH rose on enclomiphene and fell on gel. Sperm stayed normal on enclomiphene, with a marked drop on gel |
| Hohl 2025, meta-analysis [12] | Randomized trials pooling clomiphene and enclomiphene, searched through July 2024 | Total testosterone up 273.76 ng/dL vs placebo. No significant testosterone difference vs gel; LH and FSH higher than on gel |
The meta-analysis pools two distinct drugs. Its hormone estimate is not an enclomiphene-only effect and does not establish symptom relief, pregnancy outcomes or long-term safety.
The big gap is how men feel. FDA's reviewer told the 2022 committee that enclomiphene trials "did not evaluate improvement in hypogonadal symptoms or quality of life," and that they "did not demonstrate clinically meaningful improvement in symptoms or signs" [4]. A higher number on a lab report isn't the same as more energy or a stronger sex drive.
Also worth knowing: the effect wears off. In the Kaminetsky study, testosterone dropped back to pretreatment levels one month after men stopped [9]. That observation does not determine an individual’s appropriate duration or establish a permanent hormonal reset.
Fertility: sperm counts held, but no trial counted pregnancies
Keeping sperm production is the main reason men ask about enclomiphene. In the phase III trials, sperm concentration stayed in the normal range on enclomiphene, while men on testosterone gel saw a marked drop [11].
FDA doesn't treat that as proof of fertility. Its reviewer said changes in semen numbers alone aren't enough to establish efficacy, because "improvement in semen parameters does not ensure fertility" [4]. The cited trials do not establish a pregnancy benefit.
If children are in your plans, say so at the first visit. FDA's reviewer noted that approved options for secondary hypogonadism include testosterone and hCG [4]. A fertility specialist can tell you which fits. For how TRT affects sperm, see enclomiphene or testosterone.
Side effects and safety: short trials, open questions
Common side effects in the studies were headaches, hot flushes, nausea and muscle spasms. FDA's bigger concerns were heart and clotting events, higher estradiol, PSA and red-cell counts, and a lack of long-term data [4].
- Serious events in a safety study. FDA's summary lists bradycardia, chest pain, TIA, low blood pressure, atrial flutter, pulmonary embolism and deep vein thrombosis [4].
- Heart and clots vs testosterone. Per FDA's account of Europe's review, cardiac and thromboembolic events ran slightly higher on enclomiphene than on testosterone [4].
- Eyes. Cataracts showed up in animal studies. Europe's reviewers didn't find conclusive evidence in people but asked for eye monitoring [4].
- Lab changes. Higher estradiol, PSA and hematocrit. In the phase III trials, men stopped over high hematocrit and, in one case on enclomiphene, high PSA [4].
- Why men quit. Blurred vision, muscle spasm, headache and aggression led to discontinuations in Europe's review [4].
Compounded drugs aren't FDA-approved, and FDA doesn't check them for safety or quality before they're dispensed. With enclomiphene there's also no approved product to compare against, so the pharmacy that fills it matters more than usual.
FDA status: never approved, compounded while FDA decides
Enclomiphene has no FDA approval. FDA lists enclomiphene citrate in Category 1, substances under evaluation, in the cited 503A document. Its interim policy is conditional and does not establish that every preparation or seller complies [1].
For Category 1, FDA says it "does not intend to take action against a compounder" using the ingredient, provided the conditions in its guidance are met [2]. That cover lasts "until the agency decides on inclusion" on the permanent list, or until FDA removes the ingredient from Category 1 [2]. The cited active-ingredient search returned no standalone enclomiphene product [6].
| When | What happened | What it means for you |
|---|---|---|
| August 20, 2015 | FDA published notice of an advisory meeting on NDA 207959, Repros' enclomiphene capsules for secondary hypogonadism in men under 60 with a BMI over 25 [5] | The drug reached FDA as a full application |
| 2015 | Repros announced a complete response from FDA: the phase 3 design couldn't show clinical benefit, plus concerns about entry criteria, titration and lab methods, as FDA's reviewer recounted in 2022 [4] | No approval |
| 2018 | Europe's medicines agency refused marketing authorization, finding safety and efficacy not sufficiently shown, per FDA's reviewer [4] | No approval in Europe either |
| June 8, 2022 | FDA proposed that enclomiphene not go on the 503A list. Its advisers voted 4 yes, 8 no on listing it, several citing a lack of efficacy evidence [3] | The experts sided with FDA's staff |
| January 7, 2025 | FDA stopped sorting newly nominated ingredients into categories. Ingredients already in Category 1 keep the interim policy [2] | Enclomiphene's spot stays in place |
| May 14, 2026 | Still in Category 1 on FDA's current list. No final decision published [1] | Conditional interim policy; not approval |
The vote was advisory, not a final rule. The current cited categories document still lists the substance under evaluation. Conditions in the interim guidance and other applicable federal and state requirements remain relevant. A later final determination or removal from Category 1 could change this policy; do not assume a long prepaid plan guarantees continued availability [2].
The committee member who voted to list it pointed out that the USP has a monograph for clomiphene citrate, which contains enclomiphene [3]. That's about whether the ingredient can be characterized, not whether it works.

How to get enclomiphene prescribed
A qualified clinician must evaluate symptoms, repeat morning testosterone when appropriate, pituitary-gonadal function and the underlying cause. Low laboratory testosterone alone does not establish that enclomiphene is appropriate. Ask which consultation, testing and follow-up the actual program requires.
- Labs first. Expect testosterone, LH and FSH at minimum. The trials required two low morning testosterone results on separate days.
- A clinician visit. Usually an online form plus a video or message visit. Controlled-substance requirements differ from testosterone, but ordinary prescribing, telehealth and compounding requirements still apply.
- A compounding pharmacy. Confirm the actual dispensing location, its authorization for your address and the specific formulation.
- Follow-up labs. Estradiol, PSA and red-cell counts can rise, so repeat blood work belongs in the plan.
Coverage and reimbursement need confirmation for the exact product and services. Do not assume that an insurance logo, HSA/FSA message or generic-compound label means the plan will pay.
Five questions to ask before you pay
A program worth your time answers these up front. If it can't, it isn't ready for you yet.
- Is this compounded enclomiphene or off-label clomiphene, and which pharmacy makes it?
- Which labs do you need before the first prescription, and are they in the price?
- How often will you recheck testosterone, estradiol, PSA and hematocrit, and who reads the results?
- What does month two cost on my plan, and what happens to a prepaid plan if FDA ends compounding?
- If I'm trying for a baby, will you refer me to a fertility specialist?
Who enclomiphene fits, and who should look elsewhere
- Worth asking about if you're a man with low testosterone and low or normal LH, you want to protect sperm production, and you accept an unapproved compounded drug with short trials behind it.
- Look elsewhere if your testes don't produce on their own (primary hypogonadism), if you want an FDA-approved drug with a large safety trial (that's TRT), or if you have a clotting or heart-rhythm history your prescriber hasn't cleared.
- Not a fit without labs. For the secondary-hypogonadism use discussed here, diagnosis requires clinical assessment rather than symptoms or a single number alone.
Still weighing options? Enclomiphene or testosterone lays the two side by side. If needles are the sticking point with TRT, see testosterone injections or cream. Curious about the growth-hormone route? Sermorelin or testosterone covers it, and does sermorelin increase testosterone answers the obvious follow-up.

Frequently asked
- Is enclomiphene FDA-approved?
- No standalone enclomiphene product is FDA-approved in the reviewed record. The current cited 503A document lists enclomiphene citrate in Category 1; the interim enforcement policy is conditional and does not approve individual compounded products.
- Is enclomiphene legal?
- Category 1 is a conditional interim 503A policy, not blanket legal permission for every product or transaction. The2022 vote was advisory. The actual pharmacy, preparation and prescribing arrangement must satisfy applicable requirements; confirm current status before a long commitment.
- Is enclomiphene the same as Clomid?
- No. Clomid is clomiphene, a mix of enclomiphene and zuclomiphene, approved for ovulation problems in women. Enclomiphene is one of those two parts on its own, and it isn't approved.
- Does enclomiphene affect fertility?
- In trials it kept sperm counts in the normal range, while testosterone gel lowered them. The cited trials do not establish a pregnancy benefit, and FDA says improved semen numbers do not ensure fertility.
- What are the side effects of enclomiphene?
- Headaches, hot flushes, nausea and muscle spasms were common. FDA flagged heart and clotting events, higher estradiol, PSA and red-cell counts, and missing long-term data.
- How much does enclomiphene cost?
- Compare the actual medication amount and billing interval with required lab, consultation and membership fees. A longer-plan equivalent can require prepayment. The selected Strut thirty-day charge differs from its annualized monthly comparison.
